• As a Skilled Nursing Facility (SNF) provider of internal medicine services, you will lead clinical care for residents.
• Collaborate with the interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while overseeing the care of all residents.
• Manage day‑to‑day care of short‑term and long‑term residents with assistance from nursing staff and guidance from medical directors and attending physicians.
• Evaluate new‑admission visits, coordinate discharge visits, and conduct monthly stability visits for long‑term residents.
• Provide advanced care planning, perform comprehensive physical examinations, and develop and implement patient management policies and procedures.
• Prescribe necessary medications and treatments under direct management protocols for acute and chronic illness.
• Facilitate referrals to other healthcare professionals and communicate with attending physicians and specialists regarding patient care.

• Advanced Practice Licensure (APN) with a Florida license.
• Active license and DEA number.
• Master’s degree required.
• Previous experience in nursing or other medical fields.
• Ability to build rapport with patients and their loved ones.
• Ability to thrive in a fast‑paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.
