Base pay range$/yr - $/yr
Major Responsibilities- As a Skilled Nursing Facility (SNF) provider of internal medicine services, you will lead clinical care for residents.
- Collaborate with the interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while overseeing the care of all residents.
- Manage day‑to‑day care of short‑term and long‑term residents with assistance from nursing staff and guidance from medical directors and attending physicians.
- Evaluate new‑admission visits, coordinate discharge visits, and conduct monthly stability visits for long‑term residents.
- Provide advanced care planning, perform comprehensive physical examinations, and develop and implement patient management policies and procedures.
- Prescribe necessary medications and treatments under direct management protocols for acute and chronic illness.
- Facilitate referrals to other healthcare professionals and communicate with attending physicians and specialists regarding patient care.
