The Intake Clinician responds to inquiry calls, conducts initial intake assessments, provides referrals, and coordinates the admissions process according to Joint Commission, Federal and State Regulations, Oceans' Mission, policies and procedures and Performance Improvement Standards.

• Responds to inquiry calls, completes inquiry forms accurately, schedules assessments in a timely manner and documents follow-up calls made regarding pending assessments.
• Conducts assessments at facility or other locations, completing comprehensive interdisciplinary initial triage thoroughly.
• Refers patients not appropriate for facility programs to appropriate resources.
• Coordinates pre‑certification process and admissions for patients admitted to facility programs, including admission forms, CIAs, and documents pertinent UR information.
• Delivers patient services in a respectful manner that promotes dignity and patient satisfaction.
• Identifies safety needs, detects possible signs of abuse and neglect and reports to clinical staff.
• Identifies special mobility and equipment needs of patients.
• Assesses and interprets age and disability needs to determine level of care and makes appropriate referrals.
• Communicates with patients and families in a manner sensitive to age, disability and cultural needs, involving families of minors or significant others in decision making as appropriate.
• Performs other duties and projects as assigned.

Educational / Experience Requirements: Active Registered Nurse license in the state where practicing. Minimum of one year experience in Mental Health/Chemical Dependency field working with an Age/Disability Specific population served by this facility preferred.

Qualifications /

Skills:

Admission criteria knowledge for varying levels of care, medications commonly prescribed for psychiatric illness, and community resources. Interpersonal skills to develop rapport with patients, families, and referral sources. Assess for appropriate care across all age/disability needs. Knowledge of growth and development principles appropriate to the patient population. Ability to assess and interpret patient status data, identifying needs and providing referrals including age and disability related care for chronically mentally ill, adult, geriatric, and chemically dependent populations.

Knowledge of mental illness and chemical dependency and family dynamics. Basic knowledge of chronic mental illness, clinical therapy modalities. Excellent communication skills. Assessment skill demonstration. Post‑test readiness. Versatility, flexibility, and performance in stressful situations. Time management and organizational skills. Ability to work with mentally/physically disabled persons. Reading, writing, and following verbal and written instructions. Knowledge of DSM‑IV‑R diagnosis. Successful completion of CPR certification and an Oceans approved behavioral health de‑escalation program.

Work Environment: Subject to many interruptions. Occasional pressure due to multiple calls and inquiries. This position is high‑paced and stressful; must cope mentally and physically. This person may experience disagreeable odors, sights, and/or unpredictable patient behavior. Requires spending approximately 90% or more of the time inside a building with protection from weather conditions but not necessarily temperature changes.
