Who We Are - CDWI

This is a heartwarming opportunity for compassionate providers who love making a difference! We work with members throughout Wisconsin to find providers that fit each of their needs. Our goal is to help individuals be in more control of their own supports and to have a say in who is assisting them. This helps them to stay in their own homes as long as possible.

We are looking for an experienced candidate in or near the Brown Deer area to provide care to an individual living in his own home. Candidate must be reliable, dependable, and a good communicator. Schedule, tasks, and requirements for this position are listed below:

Schedule
:
Monday through Friday, 8am to 4pm

• Care is to be provided in member’s home.
• Assist with transfers (Hoyer Lift).
• Assist with feedings (G Tube, suctioning).
• Assist with bathing and getting dressed.
• Assist with toileting, when needed.
• Assist with medications, when needed.
• Above all, provide safety and companionship.

• Valid Driver's License
  -or- reliable transportation to and from shifts.
• Prior experience working with feeding tubes and Hoyer lifts required.
• Must be at least 18 years of age at the start of employment.
• Must pass background check.

* If hired, Consumer Direct Care Network (CDCN) will not be your legal employer on record. If hired by a Member or their Managing Party, your legal employer on record would be the Member. CDCN’s role is to assist the Member or Managing Party in recruiting a Direct Care Worker. The Member or Managing Party has the sole authority to hire or dismiss any potential Direct Care Worker.
