• Implement treatment plans developed by a supervising therapist using appropriate modalities, seeking consultation as necessary.
• Monitor patient responses to the treatment plan and regularly communicate patient progress and possible goal revisions to the physical therapist.
• Document services provided and patient response/progress.
• Educate patients and family/caregivers about patient deficits.
• Assist with patient discharge planning and provide information regarding appropriate selection/use of adaptive equipment and community support programs.

• Education:
  
  Associate’s degree; graduate of a program in a related discipline.
• License:
  Physical Therapy Assistant in the state of IN.
• Certification:
  Basic Life Support (BLS).
• Skills:
  
  critical thinking, decisive judgment, ability to work with minimal supervision, handle stressful environments, and take appropriate action.
