Clinical Coordinator - Clinic MA, LPN, RN

We are seeking a Clinical Coordinator to assist with all clinical needs for our [LOCATION] clinic patients.

The Clinical Coordinator plays a crucial role in coordinating patient care. This position involves proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support.

This position will be working in the [LOCATION] clinic daily. This is a full-time, salary-based 8-hour position (8:00 AM–5:00 PM), Monday–Friday.

Your Health is currently looking for a Clinical Coordinator to join our growing urgent care family. The Clinical Coordinator plays a crucial role in coordinating patient care. This position involves proficient chart preparation, communication, scheduling of patient visits, assessing vital signs and assisting with direct patient tasks, appropriate utilization and logging of chronic care management activities, and provider and patient support.

A successful Clinical Coordinator will be able to perform these essential duties accurately and efficiently. Prior experience in a similar role is crucial to the success of our team due to the fast-paced and value-based model of care.

Qualifications
  • Active CMA, LPN or RN license
  • Must have at least 3 years of experience
  • Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization
  • Strong problem-solving abilities
  • Ability to work effectively under pressure
