- Provide home visitation services to medically diagnosed high‑risk pregnant, post‑partum and inter‑conceptual women.
- Provide nurse case management to refer for support, education and community resources.
- Target populations will be provided outreach to reduce disparities in birth outcomes.
- General clinical support will be provided in the home.
- Communicate with health care providers with client status updates throughout the program.
- Must have a valid driver’s license.
- Associate degree in nursing from an accredited college or university.
- Completion of a PHN course within one (1) year of hire.
- Current Georgia Registered Professional Nurse license and CPR certification.
- Valid driver’s license.
PH Nurse 1 – Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification. Salary $51,811.56.
PH Nurse 2 – Associate degree in nursing from an accredited college or university, one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower level PH Registered Nurse 1 (HCP
190) or position equivalent. Salary $58,547.06.
PH Nurse 3 – Associate degree in nursing from an accredited college or university, one (1) year of nursing experience and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification; or two (2) years of experience at the lower‑level PH Registered Nurse 3 (HCP
192) or position equivalent. Salary $66,168.17.
- BSN.
- Previous experience in home care and/or medical case management, newborn and/or pediatric.
- Bilingual – Spanish.
- One year of Labor & Delivery experience OR One year working in a prenatal care clinic or One year experience working with moms and babies.
