You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.
Schedule is Monday–Friday, 8:00 AM – 4:30 PM with on-call opportunities and potential dedicated on-call teams for backup coverage.
