.

• Full time outpatient position
  , minimal IP consults required
• Office hours of operation:
  Monday-Friday
  • Flexible hours available to accommodate work-life balance
• Minimal Call

• Group consists of physicians and APPs that are dedicated to providing quality team-based care to adults with chronic conditions throughout Northeast Indiana and Northwest Ohio
