Registered Nurse (RN) Case Manager – Hospice About the Role

We’re seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our hospice care team. In this vital role, you’ll provide expert clinical care, emotional support, and guidance to patients and families navigating the end‑of‑life journey. You’ll coordinate care across an interdisciplinary team to ensure comfort, dignity, and quality of life for every patient we serve.

What You’ll Do
  • Manage a caseload of hospice patients, serving as the primary nurse contact for patients, families, and caregivers.
  • Conduct thorough initial and ongoing assessments to guide individualized care plans.
  • Deliver compassionate, evidence‑based nursing care focused on pain and symptom management.
  • Collaborate closely with physicians, social workers, chaplains, home health aides, and volunteers.
  • Educate patients and families on medications, comfort measures, and the hospice philosophy.
  • Document care promptly and accurately in the electronic medical record (EMR).
  • Participate in interdisciplinary team meetings and on‑call rotations as needed.
What We’re Looking For
  • Current, unrestricted Registered Nurse (RN) license.
  • Minimum 2 years of clinical nursing experience, preferably with 1+ year in hospice, home health, or palliative care.
  • Excellent communication, critical thinking, and organizational skills.
  • Proficiency with electronic charting and mobile technology.
  • Reliable transportation and valid driver’s license (if travel is required).
