Home Health Hospice Registered Nurse (Locum Tenens) – Buckhannon, West Virginia. We are seeking a compassionate Registered Nurse to provide hospice care and clinical support to patients and families in their homes.

• Serve as a Hospice Case Manager, delivering compassionate home health and hospice care
• Conduct comprehensive patient assessments and create individualized care plans
• Provide skilled nursing care, symptom management, and end-of-life support
• Collaborate with interdisciplinary healthcare team members for holistic patient care
• Maintain accurate clinical documentation using OASIS and HCHB systems
• Educate patients and families about hospice services, disease processes, and available resources

• Active RN license in West Virginia
• Minimum 2 years of experience as a Hospice Registered Nurse
• Proficiency with OASIS and HCHB (Homecare Homebase) documentation systems
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation
