Compensation: $95,000 - $130,000 per year + Incentive Visits!

In this full-time RN Case Manager role, you’ll assess clients’ health needs, provide skilled nursing care, and guide caregiver training across the Twin Cities Metro Area. You’ll experience the satisfaction of delivering quality care directly to clients in their homes, with the support of a collaborative and dedicated team. You will also move around to help with caseload fluctuations across designated territories to ensure a timely initiation of care and progress clients through their care plan.

• Deliver nursing services, client assessments, and ongoing care
• Train and mentor caregivers to uphold high standards of service
• Engage in process improvements for continuous quality enhancement
• Rotate in the on-call schedule

• Minnesota RN license
• Reliable transportation with insurance
• Proficiency in nursing assessments and client communication
• Experience in home health care is a plus
