Position: SPEECH LANGUAGE PATHOLOGIST

Speech Language Pathologist (SLP) is responsible for evaluating, developing, and implementing speech therapy services. Documents treatment sessions in accordance with policies and procedures of the department and regulatory bodies.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES
  • Completes a preliminary screening and initial evaluation for all newly referred individuals, makes recommendations as appropriate.
  • Develops treatment plan based on evaluation and assessment. Treatment plan is submitted for medical director and/or referring provider approval or certification as regulated. Modifies treatment plan as necessary.
  • Provides direct speech therapy services to persons with speech/ language needs including addressing swallowing and augmentative communication needs.
  • Recommends modified barium swallow studies as needed.
  • Documents treatments as required.
  • As a member of the interdisciplinary treatment team, participates in professional meetings as assigned, integrating services with other providers.
  • Collaborates with other team members to facilitate carryover of recommendations and continuity of care.
  • Makes recommendations for the purchase of specialized equipment and materials.
  • Lends counsel to patients, family members, and caregivers regarding the participant’s speech/ language services, including education upon discharge.
  • Provides an orientation to speech/language services to new staff and participates in in-service training.
  • Responsible for ensuring that confidentiality is strictly maintained concerning all PHI and agency-related information.
  • Maintains productivity expectations set forth by Rehab Leadership Team.
  • Functions as clinical instructor for SLP students as assigned.
  • Maintains clinical competency by attending & completing at least one continuing education class/course each year as well as maintaining CEU requirements for state licensure and professional certification.
  • Maintains at least 90% compliance on chart audits and follows required documentation guidelines & expectations set forth by the department, NYSEd, DOH, OPWDD, insurances, etc.
  • Refers patients to other disciplines as needed.
  • Maintains SCIP-R Core certification and CPR/AED certification as well as all required certifications set forth by Rehab Leadership Team.
  • Covers speech therapy services at 800 Hertel and our satellite locations as assigned.
  • Complies with all agency policies and procedures including OPWDD Choking Prevention Initiative.
  • Conducts all professional activities in accordance with New York State practice act and ASHA guidelines.
  • Other duties as assigned.

  • Plans personal time off (PTO) in advance and manages PTO according to department guidelines.
MINIMUM QUALIFICATIONS
  • Licensed and registered to practice Speech Language Pathology in New York State.
  • Must hold Certificate of Clinical Competency in Speech Language Pathology granted by the American Speech Language and Hearing Association.
  • Must meet continuing education requirements to maintain NYS licensure and ASHA certification.
SUPERVISORY RESPONSIBILITIES

This position requires supervision of a graduate student clinician or clinical fellow as assigned.
