Assisted Living Facility in the Buffalo, NY region seeking a Case Manager
Salary: $63,000–$65,000 annually

ResponsibilitiesAssist with the integration of individuals into our senior living facility.
Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility, including addressing their social, emotional, and psychological needs while following DOH guidelines.
Act as a liaison between residents, families, and other healthcare entities.
Assist with quality assurance reviews and maintain accurate, up-to-date files and case management notes.
Work closely with the entire care team to ensure a positive and safe environment for all residents.
Demonstrate compassion and a strong commitment to working with the aging
- Bachelor's degree in a human services field and one year of full-time work experience in a related field; or
- Associate's degree in a human services field and three years of full-time work experience in a related field.
- Previous case management experience required.
- Experience in Skilled Nursing, Assisted Living, and/or Memory Care required
