This is a part time position in the Admissions Department. 8am-4pm every Saturday and Sunday. Holiday rotation.

• A Licensed Practical Nurse (LPN) Coordinator handles the day to day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies.
• Communicates as a liaison between staff, patients, and outside agencies.
• Assists with maintaining records.
• May involve direct patient care.

• Assists in coordination and implementation of the patient plan of care.
• Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
• Collaborates with the interdisciplinary teams, RNs, or Team Manager.
• Coordinates and schedules patient visits.
• Provide timely documentation/data entry per Hospice time frames and deadlines.
• Communicates current health status and needs of patients to appropriate team professionals.
• Assists the team and Team Manager with other administrative functions, as necessary.

• Education/Certification
• Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
• Experience
• 1 year of nursing practice as a Licensed Practical Nurse preferred.
