Licensed Practical Nurse (LPN) Coordinator. 7:30am-3:30pm Monday-Friday. Holiday rotation.
A Licensed Practical Nurse (LPN) Coordinator handles the day to day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates as a liaison between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.
Essential Duties and Responsibilities for a Licensed Practical Nurse (LPN) Coordinator:
Assists in coordination and implementation of the patient plan of care.
Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
Collaborates with the interdisciplinary teams, RNs, or Team Manager.
Coordinates and schedules patient visits.
Provide timely documentation/data entry per Hospice time frames and deadlines.
Communicates current health status and needs of patients to appropriate team professionals.
Assists the team and Team Manager with other administrative functions, as necessary.
Qualifications for a Licensed Practical Nurse (LPN) Coordinator:
Education/Certification
Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New
York.
Experience
1 year of nursing practice as a Licensed Practical Nurse preferred.
