Triage Nurse

Position SummaryUBMD Obstetrics & Gynecology is seeking an experienced Triage Nurse (RN) to join our clinical team. The Triage Nurse serves as a key point of contact between patients and providers, ensuring high-quality, patient-centered care through effective communication, clinical assessment, education, and coordination. This role primarily involves telephone and patient portal communication, with occasional in-person support as needed.
Essential Responsibilities- Triage and Respond:
Assess patient health concerns, determine urgency, and recommend appropriate levels of care in accordance with clinical judgment, evidence-based guidelines, and NYS/UBMD protocols. - Clinical Liaison:
Communicate clinical updates between patients and providers, ensuring continuity and coordination of care. - Patient
Education:
Provide nursing advice, education, and counseling to patients and families regarding care plans, treatments, and next steps. - Medication Management:
Facilitate medication refills and authorizations per provider direction. - Prior Authorizations:
Complete insurance prior authorization requests as required. - Care Coordination:
Assist with referrals and coordination of multidisciplinary care. - Documentation:
Maintain accurate and complete patient records, documenting all communications and interventions. - Quality & Safety:
Adhere to UBMD and NYS standards of care, promoting patient safety and high-quality outcomes.
- Assist providers with procedures as needed.
- Perform venipuncture, blood collection, and specimen preparation for laboratory testing.
- Support clinical flow by performing LPN or medical assistant duties when necessary.
- Participate in other clinical or administrative tasks as assigned.
- Graduate of an accredited Registered Nurse (RN) program.
- Current New York State RN license.
- Minimum 3 years of nursing experience, preferably in an outpatient or OB‑GYN clinical setting.
- 1-3 years of triage experience in a fast‑paced environment.
- Strong proficiency in computer and phone systems; ability to manage high call volume.
- Excellent verbal and written communication skills.
- Ability to multitask and prioritize effectively in a dynamic clinical setting.
