A licensed Home Care Agency in New York City is looking for a passionate dedicated RN to help assist with Health Assessments. We work within the community to provide quality Home Care for individuals in need. Our team is made up of dedicated hard‑working & caring professionals and we are looking to add an RN that meets these high standards. We are committed to providing our clients with the best possible and most compassionate care!

• Provide initial/reassessment nursing assessments on all clients, following the guidelines set forth by the Department of Health
• Participate in audits by Agency and/or regulatory bodies.
• Ability to create a Plan of Care (POC) that will fit the client's needs.
• Conduct a minimum of 23 assessments per week.
• Ensure timely Start of Care visits in Manhattan and the Bronx.
• Travel to the Bronx and Manhattan boroughs, with occasional work in Brooklyn and Queens.
• Create and adhere to a personal schedule to guarantee timely visit completion.
• Confirm visit completion daily and submit visit paperwork within 72 hours.
• Other responsibilities as assigned.

• Must hold a valid NYS RN License
• Home Care Experience preferred.
• Excellent communication skills
• Compassionate, Caring & Understanding
• Flexibility to accommodate last‑minute visit requests.
• Bilingual preferred;
  Spanish‑speaking is required.

• On the Road

The hiring range for this position is $‑ annually. Various factors will determine final compensation such as a candidate's years of relevant work experience, skills, certifications, and location.
