Job location:

Buffalo, NY. This position is part of the New York State Office of Mental Health.

$74,193.00 per year – $94,121.00 per year (actual pay based on skills and experience).

• Providing crisis intervention, service linkages, and discharge planning.
• Developing treatment plans with individuals and the treatment team; monitoring and evaluating progress; modifying plans as needed.
• Determining discharge needs and appropriate community resources.
• Establishing and maintaining relationships with community providers to ensure continuity of care.

• Licensed Master Social Worker (LMSW) registered in New York State (eligible under the NY HELPS program).
• OR a permanent competitive or 55‑b/55‑c state service employee with at least one year in a title eligible for transfer under Section 70.1 of the Civil Service Law.

• LMSWs are eligible for shift differential pay: 7% for evening and weekend day shifts, 10% for night shifts.
• Background checks required. Maintenance of license and registration is mandatory; failure to do so may result in removal from the position.
• Applicants must not be listed as an excluded individual/entity on Federal or State Medicaid and Medicare exclusion lists.
• Appointment may require enrolling as a Medicare provider and obtaining an NPI number.
