Visiting Nurse - Facilities Registered Nurse

Your Health is seeking a Registered Nurse to service facility patients throughout the Sugar Hill, GA area. Visiting Nurses will service Assisted Living Facility and Independent Living Facility patients in care coordination with the provider within their assigned service area(s).

This is a salary-based, full-time position from 8:00 AM to 5:00 PM, Monday through Friday.

The following essential functions may be subject to change. Management may assign new duties, reassign existing duties, and/or eliminate functions.

• Admission: communication, collaboration, and coordination of care; continuous review and revision of CCM plan of care; facilitate telehealth visits with the nurse practitioner; coordinate with hospice interdisciplinary team; participate in hospice IDG meetings.
• Assisted Living & Skilled Nursing Facilities: rounding with physicians; coordinate with the patient’s health care team and other care-plan members; assess and chart observations; collaborate with nurse practitioners to meet quality measures; facilitate telehealth/tele-pharmacy visits; ensure follow-up visits are scheduled; participate in quality improvement projects; maintain current provider/specialist lists in EMR; communicate with patients, families, and caregivers; utilize company software and coaching calls;
  
  perform other duties as requested.

• Must be a licensed nurse in good standing with the appropriate board.
• Minimum of three (3) years clinical experience preferred.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively; strong written and verbal skills; basic computer knowledge; ability to lead and collaborate.
• Excellent interpersonal skills under varying conditions.
