Job Description:
1. Programme Design, Delivery & Evaluation
- Curate, conduct, and review evidence-based programmes and activities.
- Lead physical activity sessions including mobility training, strengthconditioning and functional assessments (pre/post programme).
- Ensure participant safety and maintain accurate attendance and health records.
2. Community Care, Case Management and Volunteer Management
- Engage seniors and families to build supportive relationships and identify needs.
- Conduct case management and facilitate applications for relevant social grants orschemes.
- Assist seniors with Activities of Daily Living where required.
- Assist in the Recruitment, Training, and Management of Tzu Chi Volunteers servingat Comm Care premises and Senior Volunteers assisting Comm Care
3. Administration & Stakeholder Engagement
- Prepare and monitor KPIs and grant submissions.
- Participate in Projects to support beneficiaries
