Southwest Behavioral & Health Services seeks a dedicated and empathetic Case Manager who can coordinate with people, solve problems, stay organized, and communicate effectively.

Job Preview At a Glance

The Case Manager serves as a central support within the interdisciplinary team, working primarily with adults. This staff member conducts assessments, assists members with resources within the community, and coordinates care inside and outside the agency.

Pay Rate: $17.50–$18.00 hourly.

Responsibilities
  • Intake screening and completing paperwork and referrals.
  • Completing annual behavioral health assessments, including service planning and support planning.
  • Facilitating CFTs, ARTs, and groups.
  • Identifying gaps in care for members.
  • Completing SMI and SED evaluations.
Qualifications
  • Minimum 22 years of age.
  • Bachelor’s degree in the behavioral health field plus one year of relevant behavioral health experience.
  • Combination of experience and education totaling 2 years is acceptable.
  • Valid AZ driver’s license and motor vehicle clearance.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible for within eight weeks of hire.
