Main Responsibilities:

As a valued member of the Operose Health team, your key duties will include:
Deliver comprehensive clinical medication reviews for inpatients, optimising treatment and improving patient outcomes. Provide expert prescribing, therapeutics, and medicines-related advice to clinical and non-clinical colleagues, including doctors, nurses, nursing associates, pharmacy technicians, an, As a Clinical Pharmacist at Operose Health, you will play a crucial role in delivering great primary care by improving access, outcomes, and patient experiences. Your primary responsibilities will involve delivering comprehensive clinical medication reviews, providing expert advice on prescribing and therapeutics, and leading on medicines optimisation to support patients in gaining the maximum benefit from their medications.

In addition, you will actively promote the safe, effective, and cost-conscious use of medicines in alignment with best practice and local formulary guidance. You will identify and address unmet pharmaceutical needs, ensure high standards of medicines safety and governance, and undertake deprescribing activities to minimize pharmaceutical waste. As a professional leader on the ward, you will be responsible for medicines-related matters, including risk management, clinical governance, formulary compliance, service development, and expenditure awareness.

You will support safe and timely patient discharge through effective medicines reconciliation and coordination with the wider care team, deliver high-quality, patient-facing care, and contribute to workforce development through education and training on medicines optimisation.
