VW Administrative Patient Care Navigator

An exciting opportunity has arisen for a Band 4 Administrative patient care co-ordinator tojoin Nottingham City Cares Virtual Wards service on a permanent basis working
37.5 hours per week.

We are looking for an experienced, enthusiastic, and motivated administrator to join our Virtual Wards service. A social enterprise organisation such asCityCare offers opportunities for innovative working, and you will be involved in the development of the service.

The post holder will provide a wide range of administrative support mechanisms to the team and will play a pivotal role in the overall success ofthe team.

You must be willing to participate in the management of change and be able to prioritise tasks and manage time effectively. You must also have the ability to manage your own workload with the support of your team leader and work autonomously. A thorough training package will be offered including internal courses, dependant on previous experience and you will have access to support from senior colleagues.

The hours of work will between the hours of 8am and 6pm Monday to Friday.

Job purpose

• To be an integral part of the delivery of Multi-Disciplinary Team (MDT) meetings and MDT working. Their role includes the identification of patients that may require a health or social care intervention or in complex cases an MDT discussion.
• As part of the Integrated Care system (ICS), City Care is seeking to recruit Care Navigators who will play a crucial role in establishing integrated health and social care across the City.

Dimensions

• The post holder will work within Nottingham Citys Primary Networks (PCNs ) and within the specialist services acting as the named point of information and a guide to processes for health and social care professionals The post holder will be accessible to members of the PCN via a dedicated telephone line between the hours of 8am and 6pm
• The post holder will provide co-ordination, administrative and data management support to members of multi-disciplinary team within the PCNs/services to improve joint working practices leading to more effective patient care.
• The post holder will work collaboratively with Nottingham Health & Care Point.

Key Responsibilities

To receive referral information from members of the PCNs neighbourhood (Multi-Disciplinary) team, coordinating the appropriate response based on clinical / presenting need within the agreed pathways to the most appropriate service/s

• To be responsible for the co-ordination and to liaise with all relevant statutory and voluntary sector services including the local authority responsible for the patients care to arrange the necessary support.
• To plan and organise straightforward activities whether planned or adjusted, such as staff rotas, events, meetings.
