Case Manager
We are seeking a skilled and experienced Case Manager to join our team. This role involves overseeing patient care coordination and discharge planning in an acute care environment.
Key Details- Start Date: As soon as possible
- Floating Expectation: No
- First Time Traveler: Welcomed (screening required)
- Patient Ratios: 1:25 during the day shift
- Years of Experience Required: Minimum of 2 years in acute medical care management
- Weekend Requirements: One full weekend per four weeks, based on departmental needs
- California State License: Required (pending licenses accepted if licensed by start date)
- RTO Restrictions: Maximum of 7 days
- Type of Facility: Acute Care Facility
- Scrub Color: Business Attire or Scrubs
- Shift: Day shift, 5 shifts of 8 hours each (08:00 - 16:30)
- Block Schedules: Not applicable
