Coordinate care transitions and collaborate with patients, families, providers, and post-acute services to support appropriate discharge planning. Maintain clear documentation and communicate barriers to timely, safe transitions. Candidates should be prepared to collaborate with the interdisciplinary team, maintain professional communication, and follow all applicable patient-safety, infection-prevention, and documentation standards.
