Job Opportunity

We are seeking a dedicated Medical Social Worker to join our team in an acute care facility. This position requires a professional with a strong background in social work and discharge planning, particularly in a medical setting.

Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)

Typically one full weekend per 4 weeks (subject to departmental needs) Patient Ratio: 1 Social Worker per 25 patients during the day shift

The ideal candidate will possess the following qualifications:

• Master's in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) certification.
• Minimum of 2 years of acute medical social work experience, including duties related to discharge planning.
• Experience with care coordination, collaborative care, and developing plans of care to meet patient and caregiver needs.
• Knowledge of treatment options for drug and alcohol abuse, mental health considerations, and processes for protective services.
• Ability to organize and facilitate family meetings and participate in care conferences actively.
• Familiarity with the Epic charting system is required.

This position involves working with adult and geriatric patients in a fast-paced setting with a daily census of up to 25 patients. Social workers may be required to float between nearby facilities as needed.

• Pre-orientation training of 40 hours is included.
