Registered Nurse (RN) Home Health – Full Time / Part Time (Per Diem)

As a Registered Nurse (RN) you will evaluate patients’ health, create and implement care plans, and track progress to maintain the highest standards of care. You’ll work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

• Provide nursing care within scope of practice per individualized care plan.
• Administer treatments, monitor patient conditions, and report signs/symptoms of change to Physician, Case Manager, or Director of Nursing.
• Maintain accurate clinical records and submit documentation per company policy using company‑provided device.
• Supervise LVN(s) when assigned.
• Comply with all company policies and procedures.

• Current RN license in California.
• Minimum 1 year of experience in home health or hospice care.
• Current California driver’s license.
• Ability to travel within a 15–25 mile radius.
