Position: Community Nursing Palliative Care (RN)

Part Time (22.5 hrs/week) 14 month Contract - Palliative Care Visiting Nursing for the Burlington Community

We are seeking a compassionate and experienced Community Visiting Nurse specializing in Palliative Care to join our dedicated healthcare team. This role involves providing high-quality, patient-centered care to individuals with serious illnesses in their homes, focusing on symptom management, pain relief, and emotional support. The ideal candidate will work closely with patients, families, and an interdisciplinary team to enhance quality of life and ensure comfort in alignment with the patients goals and wishes.

Acclaim Health announces the availability of one temporary (1) Established Part Time Registered Nurse position to work days starting at 8:00am in the Halton Community and Clinics with a focus on Burlington Community for a period of fourteen (14) months. As an Established Part Time, the individual will be required to forty-five (45) hours in a bi-weekly period. This position requires the individual to be available to work 3 shifts per week and 1:3 weekends in addition to the statutory holiday rotation.

PRIMARY

FUNCTIONS:
  • Provide comprehensive nursing care to palliative patients in their homes within the Halton area.
  • Organize the day, set priorities based on continuity of care, the type of care to be given and the client’s health needs.
  • Develop, implement, evaluate, and modify the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes.
  • Assess the health status of the client.
  • Administer IV therapy including central line and peripheral line maintenance.
  • Administer medications topically through natural body orifices, through tubes and by injections.
  • Handle wound care, acute and chronic, for certain types and stages of wounds.
  • Ensure that the laptop is synchronized with the network at a minimum of twice a day, at the beginning and end of your shift, in order to receive information on and update client records.
  • Attend team/POD meetings as required.
POSITION REQUIREMENTS:
  • Minimum 1 to 2 years clinical practice in a community environment preferred
  • Minimum 1 to 2 years of nursing experience preferably in palliative care, hospice, or home health.
  • Current registration with the CNO
  • Certification in Palliative or hospice care (preferred)
  • Current CPR certificate
  • Excellent interpersonal, communication and teaching skills
  • Valid driver’s license and access to a vehicle

Must be legally eligible to work in Canada
