Palliative Care Registered Nurse

Position: Acclaim Health Palliative Care Registered Nurse
Assist patients with serious illnesses as a Registered Nurse specializing in Palliative Care at Acclaim Health in Burlington, Ontario. This role is focused on patient-centered care and symptom management.

Acclaim Health is offering a part-time, 14-month contract for a skilled Community Visiting Nurse. In this important position, you will work with patients in their homes, emphasizing pain relief, emotional support, and overall quality of life. Collaborate with interdisciplinary teams and families to provide holistic care tailored to individual patient needs.

Key Responsibilities:

• Assess and provide nursing care in patients’ homes

• Implement and adjust care plans based on patient status

• Administer IV therapy and medications as required

• Perform wound care for various conditions

• Participate in team meetings and updates

Requirements:

• 1-2 years of clinical experience in palliative settings

• Registered with the CNO

• Certification in Palliative or hospice care preferred

• Valid driver’s license with vehicle access

• Strong communication and teaching skills
