Clinical Homecare Nurse Manager

Lead the way in clinical care as the Nurse Manager at Qualicare in Burlington & Oakville. This full-time role emphasizes direct involvement in care assessments and leadership for complex cases.
Qualicare is seeking a passionate Nurse Manager who is an RPN or RN to guide our homecare team. You will conduct every Aging Well Advisory assessment and take charge of building personalized care plans. Your clinical expertise is vital in managing complex cases, ensuring high standards of care, and leading nursing teams during transitions and referrals.

Key Responsibilities:

• Conduct clinical assessments for new clients personally

• Manage complex care cases, including dementia and palliative needs

• Review and update care plans quarterly for active clients

• Oversee PSW competency and incident documentation

• Facilitate collaboration with community care partners
Requirements:

• CNO registered RPN or RN in good standing

• Experience in community or home care environments

• Strong clinical decision-making skills

• Exceptional communication with families

• Gentle Persuasion Approach Trainer Certificate is a plus
