As an RPN at Para Med, you will engage with patients of all ages, addressing their palliative care needs through scheduled visits. Key Responsibilities:

• Assess patients' individualized palliative care needs

• Execute and monitor personalized care plans

• Build trust and rapport with patients through collaboration

• Maintain accurate patient records of encounters

• Provide on-call patient care support as required

Requirements:

• Registered Nurse in good standing with the CNO

• Valid driver's license and access to a vehicle

• Experience in community care preferred

• Reliability and willingness to travel required

• Passion for making a meaningful impact in patient lives
