In this Monday to Friday role, you will directly care for individuals, adhering to established plans and monitoring health needs. Collaborating closely with the Program Director and RN Case Manager, you will ensure the comfort and well-being of those you support, complying with the Massachusetts Nurse Practice Act. This position emphasizes teamwork and effective communication with both staff and the community.

Key Responsibilities:

• Provide direct nursing care according to individual plans

• Monitor health status and report changes as needed

• Liaise between the home and community services

• Administer medications and manage narcotic counts

• Collaborate with staff and document health updates

Requirements:

• Active LPN license in Massachusetts with 1+ years' experience

• Experience with multi-handicapped individuals preferred

• Valid driver's license and clean record

• Strong communication and documentation skills

• Proficient in suctioning, chest PT, and patient advocacy
