Provide community nursing care as a Registered Practical Nurse with patient assessment and care planning across home settings. Support comes from Registered Nurses for complex patients.
Para Med seeks a Registered Practical Nurse for scheduled community visits with patients of all ages. You will deliver ethical, safe care within your scope, helping patients remain comfortable position offers autonomy, independence, and work-life balance.
Key Responsibilities:- Complete scheduled community visits within your scope
- Assess individual patient needs using appropriate techniques
- Execute and monitor care plans continuously
- Adjust care plans as required
- Maintain professional patient records after encounters
- Current CNO registration in good standing
- Valid driver’s license and access to a vehicle
- Reliability and flexibility
- Willingness to travel between patient residences
- Required after-hours on-call coverage
Support patients at home through community nursing, assessment, care planning, and compassionate therapeutic relationships with Para Med.
