Community Registered Nurse Home Care

Community nursing brings scheduled patient visits, individualized assessments, and home-based care across various settings. Support patients of all ages while promoting comfort at home.

As a Registered Nurse with Para Med, you will deliver community care for patients with differing complexity and needs. You will build therapeutic relationships, monitor care plans, and maintain professional records. The role includes required on-call coverage after hours, travel within a designated area, and access to supportive nursing supervision.

• Assess individualized patient needs using appropriate clinical tools
• Execute and continuously monitor individualized care plans
• Build rapport and trust through collaborative patient engagement
• Maintain and update professional patient encounter records
• Provide scheduled visits across various patient care settings

• Registration with the CNO as a Registered Nurse
• Good standing with the College of Nurses of Ontario
• Valid driver’s license and access to a vehicle
• Community care experience considered an asset
• Willingness to travel and provide required on-call coverage
