Client Care Coordinator

Position: Thrive Group Client Care Coordinator
Serve as a Client Care Coordinator with Thrive Group in Burlington, providing essential clinical leadership and support for community-based social work. Empower clients to achieve their best quality of life.

As a Full-Time Client Care Supervisor within the Bethany Residence program, you will oversee comprehensive client care while guiding Community Service Workers and Healthcare Aides in their roles. Your primary focus will be on providing clinical assessments, crisis intervention, and strategic planning to support clients facing housing and mental health challenges. Foster collaboration among interdisciplinary teams to enhance the delivery of services and improve the outcomes for diverse individuals.

Key Responsibilities:

• Supervise staff to enhance client care delivery

• Conduct psychosocial assessments and consultations

• Implement crisis intervention plans and support care coordination

• Develop innovative therapeutic practices and programs

• Collaborate with community and healthcare partners

Requirements:

• Bachelor’s degree in Social Work essential

• Minimum of three years in a leadership role

• Current registration with OCSWSSW

• In-depth knowledge of client-centered care principles

• Valid driver’s license and successful police clearance
