Physical Therapist

Overview

At Piedmont Health Senior Care, we provide comprehensive, personalized health services tailored to seniors’ unique needs. We look for a Physical Therapist to support our Program of All-Inclusive Care for the Elderly (PACE).

The primary responsibility is to evaluate and reassess patients’ mobility and functional abilities, provide therapeutic exercises, gait training, and group exercise programming, and deliver care at the Day Center. Reports to PACE Site Director.

• Conduct initial assessments and scheduled reassessments; communicate changes to the IDT.
• Develop treatment programs, set goals, and deliver therapy.
• Review and complete assessments and discharge summaries.
• Provide manual muscle testing, sensory/perceptual testing, neuromotor testing, and evaluate assistive devices.
• Participate in IDT meetings and care planning.

• Doctorate of Physical Therapy from an accredited program.
• One year experience with frail or elderly patients preferred.
• Demonstrated clinical competency with geriatric population.
• Ability to work under pressure and meet CMS competencies.
• Excellent interpersonal, oral, and written communication skills.

• Licensed Physical Therapist in NC.
• Must be medically cleared for communicable diseases and up-to-date immunizations.

$76,424 - $102,707, plus a $5,000 Sign-On Bonus.

• Schedule:
  
  Monday through Friday, 8:00 am – 5:00 pm.
• Patient Population:
  Adult Geriatric.
