$40.00/hr – $52.00/hr

• Assessment of referred patients requiring rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Setting realistic and achievable goals for patients.
• Documenting and recording patients’ condition.
• Educating patients and families in appropriate therapy methods.
• Producing evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborating with all disciplines to provide best patient‑centered treatment and care.
• Performing all other duties assigned by the Director of Rehabilitation.
• Ensuring compliance with confidentiality, HIPAA, and Federal Resident Rights.

• Graduate of an accredited Speech Therapy college or university.
• Valid professional license and registration in the state of practice.
• Experience with long‑term care and/or geriatric care is preferred.
