The Pharmacist, Outpatient services the needs of the UVM health system ambulatory pharmacy patients. The pharmacist may work in one or more areas of the ambulatory pharmacy department, including but not limited to:
Retail, Mail Order, Home Infusion, Call Center, Specialty Pharmacy, Travel Clinic, and Transitions of Care. The pharmacists will work with multidisciplinary teams to resolve medication‑related problems, dispense medication according to prescription orders, screen and resolve medication‑related financial barriers, and counsel patients and health care professionals on proper use of medications. The Pharmacist will maintain good business practices and sound clinical judgment to ensure the success of the department and patient outcomes.

D or B.S. Pharmacy Degree from an ACPE accredited college of pharmacy. Licensed pharmacist in Vermont.

Minimum one year experience in a retail/mail/call center/specialty/home infusion pharmacy within the past five years. Minimum one year experience in a health system pharmacy preferred.
