Care Manager – Community Support Program
We are seeking a dedicated Care Manager to provide comprehensive case management and support services for adults residing in Chittenden County who are living with psychiatric disabilities and some with co-occurring substance use challenges.
This position offers a combination of administrative responsibilities and direct service. Great opportunity to support individuals in a community-based setting.
Responsibilities- Provide case management and support services to adults with psychiatric disabilities.

- Manage administrative tasks related to case files and documentation.
