Base pay range$37.50/hr - $56.25/hr
Major Responsibilities- Responsible for admission to hospice services, leading the interdisciplinary care team specific to each patient and family on your caseload.
- Obtain history and complete a physical assessment; use data to evaluate appropriateness for hospice services and level of care, and collaborate with the attending physician, medical director and hospice interdisciplinary team.
- Meet with patients and families to explain the hospice program of services and obtain informed consent for hospice care; obtain and verify the verbal certification of prognosis with the medical director and IDT.
- Review insurance/Medicare eligibility and benefits and document data to support billing efforts.
- Assess patient and family readiness; facilitate planning and implementation of care and seek consultation as needed; initiate referrals to other disciplines.
- Coordinate all services and collaborate with the interdisciplinary team to provide consistent patient care, arranging equipment, medical supplies and other necessary items as indicated.
- Communicate well with the IDT for handoff purposes, incorporating an individualized, holistic approach into the plan of care and implementing the multidisciplinary plan of treatment.
- Facilitate appropriate placement in a hospice‑contracted facility if necessary.
- Assess need of and coordinate the delivery of medications, supplies and home medical equipment.
- Demonstrate effective communication, feedback and conflict resolution skills, promoting collaboration among healthcare team members.
- Develop and provide hospice education to inpatient hospital and facility staff as directed by management.
- Act as an expert resource to hospital and facility staff regarding hospice and palliative care services.
Registered Nurse license issued by the state in which the team member practices.
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). May be obtained within 6 months based on department discretion.
A valid driver’s license issued by the Division of Motor Vehicles.
Education RequiredAssociate’s Degree in Nursing.
Experience RequiredTypically requires 2 years of direct patient care experience.

- Excellent decision-making and critical-thinking skills, with the ability to work independently and make sound judgments.

- Excellent documentation, computer, and time-management skills.
- Ability to lift up to 35 pounds without assistance; for lifts over 35 pounds, patient handling equipment and assistance of at least one other associate is expected when available.
- Must be able to travel to all areas within the Advocate Aurora service area in all weather conditions and on all types of roads.
- Must be willing to work occasional overtime if needed; required to make visits to private residences that may not be accessible to people with disabilities.
- Will be exposed to mechanical, electrical, chemical hazards, blood and body fluids; required to wear PPE and protective clothing as needed.
- Operates all equipment necessary to perform the job.
