Collaborate closely with patients, families, and the care team to deliver exceptional care and plan safe discharge processes.
Key Responsibilities:
• Conduct initial and ongoing patient assessments
• Collaborate on and revise patient care plans
• Teach families about care approaches and health management
• Delegate responsibilities to support staff effectively
• Maintain accurate patient documentation and intervention records
Requirements:
• Graduation from a recognized School of Nursing
• Active registration as a Registered Nurse with the BCCNM
• Two or more years of recent nursing experience in nephrology
• Completion of relevant postgraduate clinical coursework preferred
• Excellent interpersonal and communication skills
