The salary range for this position is CAD $44.85 - $56.02 per hour.

We are hiring for a Health Coach, Primary Care Network

Responsibilities
  • Receive referrals from primary care providers and liaise with the interdisciplinary team to determine specific behaviour problems such as physical activity, nutrition, medication adherence, and sleep; arrange client visits via face‑to‑face or virtual health visit structures.
  • Support clients through creation and implementation of personalized plans using the SMALL (specific, measurable, action‑oriented, linked‑to‑your‑life, long‑term) goals framework.
  • Provide health education to clients, families, and caregivers to increase knowledge of health status, promote health status by teaching procedures appropriate for care needs, and collaborate with internal and external providers to support the development, evaluation and enhancement of health education programs.
  • Provide lifestyle coaching based on the Small Changes health behaviour change model, utilizing motivational interviewing to address specific behavioural problems, arranging and facilitating group lifestyle coaching as required.
  • Collaborate with other primary care clinicians such as social workers, physiotherapists, dietitians, and Aboriginal liaisons to develop and provide services including wellness interventions, nutritional monitoring, goal setting, social support, and system navigation, especially for Indigenous populations.
  • Participate in program planning, development, evaluation, team‑based training, and quality improvement activities to facilitate team goal setting.
  • Prepare and maintain written and computerized records, complete program documentation, write progress notes, incident reports, and ensure referral forms are completed and sent to appropriate programs.
  • Perform other related duties as assigned.
Qualifications

Education, Training & Experience

  • Bachelor’s Degree in Psychology, Behaviour Sciences, Nutrition, Physical Education (Coaching) or a related health discipline.
  • One (1) year of recent, related experience in providing lifestyle health coaching in community and primary care environments, or an equivalent combination of education, training, and experience.

Skills And Abilities

  • Demonstrated skills in lifestyle assessment and in developing, implementing and evaluating health behaviour change programs.
  • Ability to provide individual and group small changes health coaching while maintaining fidelity to a prescribed behavioural lifestyle change intervention model.
  • Effective verbal and written communication skills.
  • Experience working in community and primary care environments with primary care providers.
  • Ability to work with a culturally diverse population, including knowledge of First Nations, Métis and Inuit health, culture and traditions.
  • Knowledge of community resources and programs, and ability to integrate client needs with existing resources.
  • Knowledge of primary prevention and chronic disease management strategies.
  • Ability to assist clients in making health behaviour changes.
  • Ability to work independently and in a team‑based environment.
  • Excellent organizational and time management skills.
  • Ability to assist clients to access online tools, resources and platforms.
  • Ability to operate related equipment and applicable software applications.
  • Training in Motivational Interviewing and Mindfulness is an asset.
  • Physical ability to perform the duties of the position.
