Fraser Health is in search of a motivated Health Coach to assist clients in achieving their health goals through tailored strategies. This role requires collaboration with health providers and an interdisciplinary team focused on holistic health education. Your expertise in lifestyle coaching and community health can significantly impact diverse populations, including Indigenous groups.
Key Responsibilities:
• Create and modify health behavior change plans
• Conduct individual and group coaching sessions
• Educate clients and families on health practices
• Collaborate with providers to enhance service delivery
• Document progress and maintain patient records
Requirements:
• Bachelor’s Degree in Nutrition or related field
• At least one year in lifestyle health coaching
• Skills in program evaluation and development
• Ability to work with diverse populations
• Familiarity with primary prevention strategies
