Primary Care Social Worker - Full-Time Relief

As a key player in promoting comprehensive client care, you will conduct psychosocial assessments and facilitate therapeutic services tailored to individual needs. Key Responsibilities:
• Lead social work assessments and treatment planning
• Support interdisciplinary care coordination and client engagement
• Advocate for clients and facilitate access to community resources
• Document assessments and maintain client records accurately
• Participate in committees to improve service delivery
Requirements:
• Valid registration with the BC College of Social Workers
• Minimum two years of experience in community health
• Ability to manage complex client situations
• Knowledge of relevant healthcare legislation
• Proficiency in communication and organizational skills
