Interdisciplinary Community Health Worker

Become a key player as a Community Health Worker at LAMP Community Health Centre. Focus on client intake and health promotion while serving diverse populations in Etobicoke.

In this position, you'll report to the Health Promotion & Community Support Manager.

Your role includes conducting assessments, facilitating community programs, and collaborating with interdisciplinary teams to enhance health service accessibility. You will work through an anti-oppression lens to promote health equity and improve outcomes for priority populations.

Key Responsibilities:

• Conduct comprehensive client assessments

• Facilitate group health programs and lifestyle workshops

• Build partnerships for community outreach

• Support program communication and awareness

• Manage data collection for program evaluation

Requirements:

• Post-secondary education in a relevant field

• 3+ years of experience in outreach or community health

• Strong communication and facilitation skills

• Knowledge of health equity frameworks

• Familiarity with Etobicoke communities is beneficial
