As a coordinator, you will oversee the critical process of transitioning patients from acute to community care. Collaborate closely with other health professionals, assess clients' needs, and develop tailored transition plans.
Your role will also involve providing guidance on policy developments and enhancing communication strategies during discharge.
Key Responsibilities:
• Lead patient transitions from acute care environments
• Collaborate with interdisciplinary teams for effective discharges
• Coordinate referrals based on clinical assessments
• Maintain comprehensive client records and documentation
• Educate families on community support systems available post-discharge
Requirements:
• Master’s degree in Health Sciences or equivalent
• Three years of experience in healthcare settings
• One year focused on discharge planning
• Strong command of communication and organizational skills
• Demonstrated ability to work independently and collaboratively
