Location: New Westminster, BC

Status: Full-time, Contract - till March 17, 2028; existing vacancy

Reports To: Manager, Healthcare Operations

Compensation: $38 - $42/ hour

This is an opportunity to play a pivotal role in an innovative Hospital-to-Home transition program that improves outcomes for individuals adjusting to vision loss. You will work alongside healthcare providers, rehabilitation specialists, and community partners to help clients regain independence, access critical supports, and successfully navigate life beyond hospital discharge. The role offers meaningful client impact, strong interdisciplinary collaboration, and the opportunity to help shape innovative approaches to integrated rehabilitation care.

The Clinical Service Coordinator is responsible for supporting individuals experiencing vision loss as they transition from hospital to home and community-based rehabilitation services. Serving as the primary point of contact from referral through service delivery, this role provides case management, care coordination, system navigation, and client advocacy to ensure individuals receive timely access to rehabilitation, healthcare, and community supports.
