Detailed Overview

Provides speech language pathology services in accordance with the College of Health and Care Professionals of BC standards of practice, Fraser Health mission, values and speech language pathology acute care standard, and in collaboration and partnership with the interprofessional care team in the provision of patient and family centered care. For clients requesting outpatient rehabilitation services, provides initial client assessment, consultation and intervention based on client needs.

Reviews referrals, determines initial services required and utilizing established standards and protocols, prioritizes and confirms referrals as appropriate. Prepares and maintains client records and provides data and statistics. Participates in approved speech language pathology and interprofessional research, special projects, and program quality and safety activities.

Responsibilities
  • Provides speech language pathology assessment of the potential client's communication, speech and swallow functional abilities, needs, and goals by methods such as reviewing records, gathering information, interviewing the patient, identifying patient and speech language pathologist expectations, selecting and applying evidence-based assessment methods, and consulting with members of the health care team.
  • Analyzes the potential client's needs and develops an evidence-based, outcome-focused intervention strategy, utilizing assessment findings.
  • Participates in the interprofessional care planning process for clients by reviewing, prioritizing, and processing requests for outpatient rehabilitation services from potential clients and other health care professionals. Determines initial services required and utilizing established standards and protocols, prioritizes referrals based on client needs and outpatient clinic capacity; accepts and confirms referrals where appropriate.
  • Participates in maintaining seamless quality client care by screening for priority of intervention, redirecting to appropriate rehabilitation services, and identifying and facilitating referrals to appropriate health care/community programs.
  • Educates clients/families/caregivers/healthcare professionals regarding outpatient rehabilitation intake policies, procedures and processes, general rehabilitation resources and the availability and/or eligibility for services.
  • Maintains a variety of records including client files by gathering information, entering data into computerized databases and/or documenting information into approved formats. Compiles statistics, summarizes information into written reports, and prepares correspondence and reports as requested.
  • Acts as a speech language pathology resource by sharing speech language pathology information to other members of the health care team and attends meetings as a representative of speech language pathology.
