Step into the role of Rehabilitation Assistant at Fraser Health, collaborating with therapists to deliver client-centered care in home and community settings. Support rehabilitation by implementing and monitoring individualized treatment plans.
In this dynamic position, you will engage with both clients and therapists to promote recovery and independence through tailored treatment strategies. Your responsibilities will include observing progress, adapting care plans based on feedback, and providing individual and group activities that enhance client skills. Building strong relationships is key to your effectiveness in this role.
Key Responsibilities:
• Implement treatment and rehabilitation programs effectively
• Document changes and progress in client behavior
• Adapt activities based on individual client needs
• Support clients in gaining functional independence
• Maintain open communication with therapy teams
Requirements:
• Completion of a Rehabilitation Assistant Program
• A minimum of one year of clinical experience
• Valid BC Driver's license for job-related travel
• Physical ability to perform job functions
• Familiarity with psychosocial rehabilitation principles
