You will provide individualized, patient-centered pharmacy care in both inpatient and outpatient settings, including Oncology. Under the guidance of the Director of Pharmacy, your responsibilities will encompass various aspects of medication management and support staff involved in drug distribution and education.
Key Responsibilities:
• Deliver patient-centered pharmacy services for diverse clinical needs
• Participate in medication management for inpatient and outpatient care
• Provide education and training on drug use and safety
• Support team members in the medication use process
• Ensure compliance with provincial guidelines and professional standards
Requirements:
• Bachelor of Science in Pharmacy or equivalent qualification
• Current Ontario College of Pharmacists registration
• 2+ years of hospital pharmacy experience preferred
• Proficient in drug information and pharmacokinetics
• Strong critical thinking and communication skills
